Do probiotics help your skin? Modestly and specifically: randomised trials show oral probiotics can lower inflammatory markers and improve hydration in people with rosacea or acne, as an adjunct to medical treatment over 12+ weeks — and the strain matters far more than the CFU count. Fermented foods are the sensible starting point. A trendy idea with more real evidence behind it than you might expect — and one where the label detail that matters is almost never the one being advertised.

What is the gut–skin axis, actually?

The phrase gets used loosely, so here is the substance behind it.

A Danish nationwide cohort study of 49,475 rosacea patients found significantly higher rates of small intestinal bacterial overgrowth, irritable bowel syndrome and inflammatory bowel disease than in healthy controls. That is a large population and a consistent association — the sort of finding that turns a hypothesis into a research programme.

  • Intestinal permeability. A compromised gut lining allows bacterial products into circulation, contributing to low-grade systemic inflammation.
  • Immune signalling. In rosacea specifically, researchers point to overactivation of TLR2 and NF-κB pathways — the same inflammatory switch that recurs throughout this library.
  • Depleted protective species. Reduced Lactobacillus and Bifidobacterium populations are repeatedly observed.

What the trials found

Rosacea. A randomised controlled trial of 60 patients gave everyone two weeks of doxycycline, then three months of either a probiotic, a placebo, or nothing. The probiotic group showed significantly lower physician assessment scores, reduced TNF-α, improved stratum corneum hydration, and decreases in KLK5 and IL-8 — two markers closely tied to rosacea inflammation.

Rosacea again. An Italian trial of 38 patients with papulopustular rosacea used oral Lactobacillus reuteri for 12 weeks, reporting a significant reduction in inflammatory lesion count — a mean change of −5.3 lesions — and lower erythema scores.

Acne. A double-blind randomised trial found improved clarity scores after 12 weeks alongside decreases in the inflammatory cytokines IL-6 and TNF-α.

Note what these have in common: 12 weeks or more, and mostly alongside conventional treatment rather than instead of it. Probiotics performed as an adjunct.

Strain is everything — and almost nobody tells you

This is the most useful thing on this page.

“Probiotics” is not one thing. Effects are strain-specific, and a result for Lactobacillus reuteri tells you very little about a different Lactobacillus, let alone a generic blend.

Most supplements advertise a colony-forming-unit count in the billions, because a big number looks impressive. Far fewer name the specific strains — and the strain is what the research was done on. A bottle boasting 50 billion CFU of unspecified species is not equivalent to a trial dose of a studied strain.

If you are going to buy one, look for named strains with published research, not the largest number on the shelf.

How strong is the evidence, really?

Promising and genuinely fragmented. A 2025 systematic review and meta-analysis of microbial interventions in inflammatory skin disease described the current evidence as fragmented, with considerable heterogeneity limiting definitive conclusions.

Reviews of the acne and rosacea literature reach a similar place: encouraging findings, but standardised trials and personalised approaches are still needed before microbiome strategies can be properly integrated into dermatology.

The trials are also small — 38 patients, 60 patients — and studied medical populations receiving medical treatment. None of that tells you what a probiotic will do for generally healthy skin.

Start with food

Yoghurt, kefir, kimchi, sauerkraut, miso and other fermented foods deliver live cultures alongside fibre and nutrients, at a cost and risk profile that supplements cannot match. For most people this is the sensible starting point, and it is what we would suggest before a capsule.

Probiotics are generally well tolerated. Mild bloating or gas in the first week or two is common as gut populations shift. Anyone significantly immunocompromised should speak to their doctor first — this is one where that caveat genuinely matters.

Do probiotics actually help your skin?

There is more real evidence than the trend suggests. Randomised trials have shown oral probiotics can lower inflammatory markers and improve skin hydration in people with rosacea or acne — but almost always as an adjunct to conventional treatment, over 12 weeks or more, not as a standalone fix. Their effect on generally healthy skin is far less established.

Are probiotic supplements or fermented foods better for skin?

How long do probiotics take to affect skin?

In the trials that showed a benefit, participants took a named strain daily for at least 12 weeks — usually alongside prescription treatment — before hydration and inflammatory markers changed. Expect nothing in the first month, and do not judge a strain on a two-week trial. If nothing has shifted after three months, it is probably not the lever for your skin.

Do topical probiotic skincare products work?

The evidence is thinner than for oral probiotics. Most 'probiotic' creams contain lysates or ferments (non-living bacterial fragments) rather than live cultures, and human trials are small. They are generally gentle and may support the barrier, but treat the label as marketing until the research catches up. A calming facial and fragrance-free home routine do more for reactive skin today.
